Mục tiêu: Đánh giá sự thay đổi chỉ số dinh dưỡng tiên lượng (prognostic nutritional index - PNI) trước và sau ghép thận, đồng thời khảo sát mối liên quan với một số đặc điểm lâm sàng và cận lâm sàng trong 3 tháng đầu sau ghép. Phương pháp nghiên cứu: Nghiên cứu mô tả theo dõi dọc trên 101 người bệnh (NB) ghép thận tại Bệnh viện Quân y 103 từ tháng 7/2025 - 7/2026. Các xét nghiệm được thực hiện ở thời điểm trước ghép (T0) và sau ghép 3 tháng (T3). Các biến cố sau ghép được theo dõi trong 3 tháng đầu sau ghép. Kết quả: PNI (T3) tăng từ 48,79 ± 5,45 lên 52,27 ± 4,93; thấp hơn ở nhóm tuổi ≥ 40, nhóm sử dụng thuốc ức chế miễn dịch (anti-thymocyte-globulin - ATG), nhóm có thiếu máu ở T3, nhóm có nhiễm trùng trong 3 tháng đầu sau ghép, nhóm có mức lọc cầu thận (MLCT) < 60 mL/phút/1,73m². PNI (T3) < 50 điểm liên quan đến khả năng xuất hiện thiếu máu và nhiễm trùng cao hơn lần lượt là 3,96 lần và 4,14 lần. Khi phân tích PNI dưới dạng biến liên tục, mỗi 1 điểm PNI sau ghép tăng liên quan với giảm OR thiếu máu là 21% và giảm OR nhiễm trùng là 16%. Kết luận: PNI cải thiện rõ rệt ở T3. PNI sau ghép có mối liên quan đến tuổi, thuốc dẫn nhập và một số biến cố sớm sau ghép như thiếu máu, nhiễm trùng.
