JOINT ULTRASONOGRAPHY IMAGING OF PATIENTS WITH STAGES 0 - 1 OSTEOARTHRITIS ACCORDING TO KELLGREN – LAWRENCE GRADING

Abstract

Study on 157 patients with stage 0 - 1 knee osteoarthritis (KOA)  according to the Kellgren - Lawrence (K/L) grading who were examined and treated at the Rheumatology Department, Military Hospital 103 from September 2023 to April 2024 shows that the patients’ average age in the study group was 58,78 ± 7,80, the proportion of female patients was higher than male patients (67,52% compared to 32,48%), the average body mass index (BMI) was 22,56 ± 1,62, manual labor workers accounted for the majority (82,17%), single joint OA accounted for 67,52%, OA of both joints accounted for 32,48%. On the X-rays, osteophytes in the medial tibial plateau accounted for 35,66%, osteophytes in the lateral tibial plateau accounted for 26,11%, stage 1 OA (K/L) accounted for 91,72%, stage 0 OA (K/L) accounted for 8,28%. Joint ultrasound revealed 52,22% of patients had cartilage damage, grade III cartilage lesions accounted for the highest proportion (57,33%), the average cartilage thickness measured at inner condyle, outer condyle, and intercondylar notches were respectively ( 2,16 ± 0,25; 2,16 ± 0,24; 2,23 ± 0,25); osteophytes at the medial tibial plateau accounted for 33,75%, lateral tibial plateau osteophytes accounted for 24,47%; knee effusion accounted for 89,8%, the average thickness of the fluid layer was 5,19 ± 2,14, grade I effusion accounted for the highest proportion (84.07%), serous effusion was predominant (91.08%); The rate of synovial membrane hypertrophy was 20,38%, 26,11% of patients had periarticular tendonitis, 17,83% had Baker's synovial cyst, effusion + synovial hypertrophy accounted for 20,38%. Ultrasound should be included as an initial screening method and should be applied routinely for diagnostic and therapeutic purposes.
