Abstract

Objectives: To evaluate the effectiveness of HDF-online (online hemodiafiltration) in reducing plasma β2-microglobulin (β2M) levels in maintenance hemodialysis (MHD) patients. Methods: A descriptive cross-sectional study with a pre–post comparison within the same patients was conductedon 67 MHD patients at the Nephrology and Dialysis Department, Military Hospital 103 from July 2025 to April 2026. β2M was quantified at two time points: Before and immediately after HDF-online. Results: Plasma β2M concentrations decreased significantly from a median of 27.41 mg/L (IQR: 24.98 - 29.51) to 10.68 mg/L (IQR: 8.59 - 12.60), with a mean absolute decrease of 16.86 ± 4.54 mg/L and a mean percentage decrease of 60.86 ± 10.44%. β2M reduction rate was higher in the group with Kt/V ≥ 1.4, albumin ≥ 38 g/L, preserved urine output, without diabetes mellitus, and dialysis vintage < 5 years. Conclusion: HDF-online significantly reduced β2M levels in MHD patients. The β2M reduction ratio (β2M-RR) was associated with Kt/V, serum albumin, residual urine output, diabetes mellitus, and dialysis vintage.
