TREATMENT OUTCOMES OF FIRST-LINE OSIMERTINIB IN ELDERLY PATIENTS WITH ADVANCED EGFR-MUTANT NON-SMALL CELL LUNG CANCER

Abstract

Objectives: To evaluate the efficacy and safety of first-line osimertinib in elderly patients with advanced EGFR-mutant non-small cell lung cancer (NSCLC). Methods: This retrospective-prospective descriptive study included 51 patients aged ≥ 65 years with stage IIIC-IV NSCLC harboring EGFR mutations, treated with first-line osimertinib at K Hospital from May 2019 to May 2025. Results: The objective response rate (ORR) was 82.4%, and the disease control rate (DCR) was 96.1%. Among patients with brain metastases, ORR was 76.2%, and DCR was 100%. Median progression-free survival (PFS) and overall survival (OS) were 26.7 and 46.8 months, respectively. The 3-year survival rates were 45.4% for PFS and 76.9% for OS. No independent prognostic factors for OS were identified in multivariable analysis. Adverse events were mainly grade 1 - 2, with no grade 3 - 4 toxicities observed; one case of grade 1 interstitial lung disease was reported. Conclusion: First-line osimertinib demonstrated high efficacy and good tolerability in elderly patients with EGFR-mutant advanced NSCLC.
