FIRST-LINE TREATMENT OUTCOMES OF ATEZOLIZUMAB PLUS BEVACIZUMAB IN PATIENTS WITH UNRESECTABLE HEPATOCELLULAR CARCINOMA: SINGLE-CENTER REAL-WORLD DATA FROM VIETNAM, FROM 2020 TO 2026

Abstract
Objectives: To evaluate the treatment outcomes of atezolizumab plus bevacizumab as first-line therapy in patients with unresectable hepatocellular carcinoma (HCC). Methods: A retrospective cohort study with longitudinal follow-up was conducted on 118 patients who received first-line atezolizumab plus bevacizumab at Vietnam National Cancer Hospital between February 2020 and February 2026. Results: The mean age was 57.5 years; 92.4% of patients were male, and 65.3% had hepatitis B virus infection. Portal vein thrombosis and extrahepatic metastases were observed in 63.6% and 56.8% of patients, respectively. The median follow-up duration was 16 months. Median overall survival and progression-free survival were 16.2 months and 5.2 months, respectively. The objective response rate was 65.3%, and the disease control rate was 67.8%. 20.3% of patients had at least one adverse event; elevated liver enzymes and gastrointestinal bleeding were reported in 11.0% and 5.1% of patients, respectively. Conclusion: Atezolizumab plus bevacizumab demonstrated promising efficacy with acceptable safety as a first-line treatment for patients with unresectable HCC.
