Abstract

Objectives: To identify the microbial etiology in hospitalized acute exacerbations of chronic obstructive pulmonary disease (AECOPD) using multiplex real-time PCR (mRT-PCR) in the context where sputum culture still has many limitations, and to describe the distribution of microbial agents and analyze factors associated with coinfection in patients with AECOPD. Methods: A cross-sectional descriptive study was conducted on 106 patients with AECOPD and acute lower respiratory tract infection who were treated at Can Tho Central General Hospital 2024 - 2026. Results: The microbial pathogen detection rate was 92.5%. The most common pathogens were Streptococcus pneumoniae (32.1%), Influenza A virus (20.7%), Klebsiella pneumoniae (20.7%), and Escherichia coli (18.9%). Conclusion: The coinfection rate was high (54.7%), mainly involving bacteria-bacteria and bacteria-viral coinfections. The risk of coinfection was higher in patients with fever, a history of hospitalization within the previous 3 months, and elevated CPR levels.
