OUTCOMES OF THE BENTALL PROCEDURE IN PATIENTS WITH MARFAN SYNDROME AT HUE CENTRAL HOSPITAL

Abstract

Objectives: To evaluate the early and mid-term outcomes of the Bentall procedure in patients with Marfan syndrome. Methods: A retrospective, descriptive case series study without a control group was conducted on 54 patients with Marfan syndrome presenting with aortic root aneurysm or aortic root dissection combined with severe aortic regurgitation. All patients underwent the Bentall procedure at Hue Central Hospital between 2012 and 2022. Results: The mean age was 34.2 years (range:  18 - 60), with males accounting for 63.0%. Preoperative heart failure status according to the New York Heart Association (NYHA) classification showed that 87.0% of patients were class II and 13.0% were class III. Ascending aortic aneurysm was observed in 88.9% of cases, while 11.1% had aortic dissection. The mean aortic root diameter was 56.3 ± 6.5mm. Concomitant procedures included mitral valve replacement, mitral valve repair, and tricuspid valve repair in 25.9%, 27.8%, and 33.3%, respectively. The mean cardiopulmonary bypass time was 181.1 ± 15.4 minutes, and the aortic cross-clamp time was 129.6 ± 14.1 minutes. The mean duration of mechanical ventilation was 19.9 ± 4.5 hours. Postoperative complications included arrhythmias, acute kidney injury, and neurological complications, with rates of 29.6%, 27.8%, and 7.4%, respectively. No patients required reoperation for bleeding, and no mortality was recorded during the follow-up period. Conclusion: The Bentall procedure in patients with Marfan syndrome is safe and provides favorable outcomes with a low rate of complications and adverse events.
