EFFICACY OF INTRAVENOUS THROMBOLYSIS IN PATIENTS WITH ACUTE ISCHEMIC STROKE OF UNKNOWN ONSET TIME BASED ON DIFFUSION-WEIGHTED IMAGING - FLUID-ATTENUATED INVERSION RECOVERY MISMATCH ON BRAIN MAGNETIC RESONANCE IMAGING

Abstract
Objectives: To evaluate the efficacy of intravenous thrombolysis in patients with acute ischemic stroke of unknown onset with diffusion-weighted imaging - fluid-attenuated inversion recovery mismatch (DWI-FLAIR mismatch) on brain magnetic resonance imaging (MRI). Methods: A retrospective, cross-sectional descriptive study was conducted in 40 patients with acute ischemic stroke of unknown onset with DWI-FLAIR mismatch on brain MRI who received intravenous thrombolysis at the Stroke Department, Military Hospital 103, from August 2025 to March 2026. Results: The study included 22 males (55%), with a mean age of 67.5 ± 12.4 years. The GCS (Glasgow Coma Scale) and MRC (Medical Research Council) scores increased significantly at discharge compared with admission (p < 0.05). The National Institutes of Health Stroke Scale (NIHSS) score progressively decreased at 1 hour, 24 hours after intravenous thrombolysis, and at discharge (p < 0.05). Following treatment, 30/40 patients demonstrated an NIHSS score reduction of more than 4 points at discharge, and 72.5% of patients achieved an mRS score of 0 - 1. There were no in-hospital deaths, serious systemic hemorrhage occurred in 2 patients, and 3 patients developed intracranial hemorrhage. Conclusion: Intravenous thrombolysis guided by DWI-FLAIR mismatch on brain MRI is effective in patients with acute ischemic stroke of unknown onset. Further studies are implemented to evaluate long-term outcomes in this patient group.
