FACTORS ASSOCIATED WITH POOR CLINICAL OUTCOMES IN PATIENTS WITH ANTERIOR CIRCULATION ACUTE ISCHEMIC STROKE FOLLOWING SUCCESSFUL MECHANICAL THROMBECTOMY

Abstract

Objectives: To identify factors associated with poor clinical outcomes in patients with acute ischemic stroke who achieved vascular recanalization by mechanical thrombectomy. Methods: A retrospective cohort study was conducted on 266 patients with acute ischemic stroke secondary to large vessel occlusion who underwent endovascular treatment and achieved successful reperfusion (mTICI 2b - 3), admitted to the Cerebrovascular Diseases Department of People’s Hospital 115, from December 2024 to July 2025. Demographic, clinical, imaging, and procedural characteristics were collected. Clinical outcomes were assessed using the modified Rankin Scale (mRS) at 90 days. Multivariable logistic regression analysis was performed to identify independent factors associated with poor clinical outcomes. Results: The proportion of patients with poor clinical outcomes (mRS 3 - 6) at 90 days was 62.4%. Independent factors associated with poor clinical outcomes included male sex, advanced age, smoking, and higher NIHSS scores at admission or at 24 hours post-procedure. Conclusion: Poor clinical outcomes remain common despite successful vascular recanalization. Identification of these key prognostic factors may assist in optimizing patient selection and improving treatment strategies.
