Clinical features and some paraclinical features in patients with benign paroxysmal positional vertigo

Abstract

Objective: Describe the clinical characteristics and some paraclinical characteristics of patients with benign paroxysmal positional vertigo (BPPV). Subjects: 40 BPPV patients diagnosed and treated at the 103 Military Hospital from October 2023 to June 2024. Method: Cross sectional - prospective. Results: The average age of BPPV patients is 56 ± 13.68 years old, the female: male ratio is 2.1: 1. The duration of vertigo in BPPV patients is 50.1 ± 45 seconds. Rotational vertigo was predominant, accounting for 95% of BPPV cases. The onset of the disease was mainly right after waking up, happening in 60% of cases. Common accompanying symptoms were nausea (92.5%), vomiting (55%) and headache (32.5%). Posterior canal BPPV accounted for 72.5%. Latency time of the nystagmus phase of the Dix Hallpike maneuver and Supine Roll maneuver was 3.08 ± 2.1 and 3.45 ± 2.11 seconds, nystagmus time was 21.66 ± 16.04 and 33.18 ± 33.34 seconds, vertigo time during maneuvers was 24.69 ± 15.6 and 40.55 ± 49.1 seconds, respectively. Blood test results, brain CT scan, and brain MRI of BPPV patients were within normal limits. Conclusion: BPPV is a common disease in postmenopausal women. Vertigo attacks are usually short < 1 minute, mainly a feeling of rotational vertigo, often appear after the patient wakes up. Common accompanying symptoms are nausea, vomiting and headache. Posterior canal BPPV is the most common type. Paraclinical tests have normal results in patients with BPPV.
