CHARACTERISTICS OF ORGAN DYSFUNCTION IN SEVERE BURN PATIENTS WITH COMPLICATIONS OF SEPTIC SHOCK

Abstract

Objectives: Evaluation of organ dysfunction in severe burn patients with complications of septic shock. Subjects and methods: The study describes 55 episodes of septic shock in 38 severe burn patients (16-60 years old) treated at the Intensive Care Unit, Le Huu Trac National Burn Hospital from January 2023 to June 2024. Results: At the time of diagnosis of septic shock: arterial blood lactate concentration had increased (2.6 mmol/L); 100% of shocks had low systemic vascular resistance index (SVRI); 12 shocks showed signs of AKI (accounting for 21.82%). All cases of septic shock had coagulation disorders, mainly mixed coagulation disorders (60.82%). 80% of shocks had respiratory failure, followed by liver failure (65.45%). The second shock group had significantly more AKI and number of failed organs than the first shock group (AKI: 41.18% vs. 13.16%; p < 0.05/number of failed organs: 1.94 ± 0.22 vs. 1.32 ± 0.14, p < 0.05). Multivariate analysis showed that SOFA score at the time of diagnosis of septic shock independently correlated with shock escape in severe burn patients (p < 0.05). Conclusion: All septic shocks in severe burn patients cause a decrease in SVRI and coagulation disorders. SOFA score at the time of diagnosis of septic shock independently correlated with shock escape in severe burn patients.
