STUDYING THE MORTALITY PROGNOSTIC VALUE OF THE PLATELET TO LYMPHOCYTE RATIO AT ADMISSION IN SEVERE TRAUMATIC BRAIN INJURY PATIENTS

Abstract

Objectives: To determine the mortality prognostic value of the platelet to lymphocyte ratio at admission in severe traumatic brain injury (TBI) patients. Methods: Prospective study was enrolled on 62 severe TBI patients who was admitted to the surgical intensive care unit of Military Hospital 103. All patients were critical resuscitated and cerebral edema management with surgical indication or not. Clinical and subclinical parameters were collected at the time of admission and discharge. The data were encoded and analyzed according to statistical methods. Results: Most of severe TBI patients were 20-40 years old (30.6%), mainly men (77.4%), and the main cause was traffic accident (72.6%). The mortality group had a statistically significant higher platelet to lymphocyte ratio than the survival group (p<0.05). The mortality group had a higher platelet to lymphocyte ratio than the survival group statistically significant (p<0.05). The platelet to lymphocyte ratio was strongly inversely correlated with lymphocytes at admission (rs = -0.898; p<0.001), moderately inversely correlated with INR (rs = -0.326; p=0.020). The area under the curve of the platelet to lymphocyte ratio predict mortality is 0.675. Multivariable logistic regression model based on age, Glasgow score at admission and Lactate/Albumin ratio had a higher mortality prognostic significance than platelet/lymphocyte ratio alone with an area under the curve of 0.835. Conclusions: Platelet to lymphocyte ratio was meaningful in predicting mortality with an area under the curve of 0.675. Multivariable logistic regression model based on platelet to lymphocyte ratio, lactate, Glasgow score at admission and had a higher mortality prognostic significance than the platelet to lymphocyte ratio alone with an area under the curve of 0.835.
