Abstract

Objectives: To evaluate the changes of interleukin-6 and interleukin-10 after free-opioid anesthesia for colorectal surgery.


Methods: 98 patients undergoing colorectal resection surgery were randomly divided into two groups: OA group, anesthesia using opioids (n = 49), intraoperative pain control with fentanyl and FOA group, free opioid anesthesia (n = 49) intraoperative pain control with continuous intravenous infusion of lidocaine, ketamine combined with epidural analgesia with levobupivacaine. Comparison of serum interleukin-6 and interleukin-10 concentrations before surgery (T0), 1 hour after surgery (T1) and 24 hours after surgery (T2) between the FOA and OA groups.


Results: Median IL-6 concentration at time points T0, T1, T2 in OFA group were 188.9; 93.4; 72.3 pg/ml and group OA were 187.6; 180.5; 188.5 pg/ml respectively. The difference about IL-6, IL-10 concentration between the two groups at each time point was not statistically significant (p > 0.05). The median IL-10 concentration value of the two groups at T1 (53.3; 64.2 pg/ml) and T2 (10.3; 13.6 pg/ml) was higher than that at T0 with p < 0.05. IL10 concentration was highest at T1 time point in two groups (53.3 and 64.8 pg/ml). IL10 concentrations at different times point between two groups was not statistically significant, p>0.05. The IL-6/IL-10 ratio at T1 and T2 was lower than T0 in both groups (p < 0.001).


Conclusion: After free opioid anesthesia, IL-6 levels gradually decreased after surgery while IL-10 increased and was highest at 1 hour after surgery. The ratio of IL-6/IL-10 after surgery decreased with statistical significance compared to before anesthesia.
