Abstract
Objectives: To investigate the clinical and paraclinical symptoms of unilateral vocal fold paralysis and thyroarytenoid muscle palsy, and to evaluate the results of surgical injection of autologous fat and connective tissue into the vocal folds to treat glottic insufficiency caused by these conditions. Methods: A longitudinal, descriptive study was conducted on 55 patients with glottic insufficiency caused by unilateral vocal fold paralysis and thyroarytenoid muscle paralysis, who underwent autologous fat and connective tissue injection into the vocal folds at Gia Dinh People's Hospital from June 2023 to December 2025. Results: The procedure was performed in 49 patients with unilateral vocal fold paralysis and 6 patients with thyroarytenoid muscle paralysis. Postoperative outcomes demonstrated significant improvements in voice quality, as assessed by the Voice Handicap Index, acoustic voice analysis, and laryngoscopy. The rate of voice improvement was 94.6%. Conclusion: Autologous fat and connective tissue injection into the vocal fold is a safe, effective, and stable treatment for glottic insufficiency. Glottic insufficiency due to thyroarytenoid muscle paralysis is a rare condition that is easily overlooked; therefore, accurate recognition and diagnosis are essential for appropriate management.
