SURVIVAL OUTCOMES AND PROGNOSTIC FACTORS IN PATIENTS WITH STAGE IV THYMIC CARCINOMA RECEIVING FIRST-LINE PACLITAXEL-CARBOPLATIN

Abstract

Objectives: To evaluate progression-free survival (PFS), overall survival (OS), and independent prognostic factors; and to compare outcomes between the de novo and post-surgical relapse (PSR) groups in patients with stage IV thymic carcinoma receiving first-line Paclitaxel-Carboplatin (PC). Methods: A descriptive study was conducted on 59 patients with stage IV thymic carcinoma, ECOG performance status 0 - 1, treated with first-line PC at Vietnam National Cancer Hospital and the National Lung Hospital, from January 2016 to December 2025. Kaplan-Meier survival analysis and log-rank testing were used to compare PFS and OS between the two groups; multivariate Cox regression was used to identify independent prognostic factors; data were processed using SPSS version 22.0. Results: The overall median PFS (mPFS) was 8.5 months (95%CI: 6.2 - 10.8), and the overall median OS (mOS) was 24.2 months (95%CI: 19.8 - 29.5). The PSR group demonstrated significantly longer mPFS and mOS compared to the de novo group (11.6 vs. 8.1 months; 30.3 vs. 20.8 months; p = 0.015). In multivariate analysis, completing 5 - 6 cycles (PFS: HR = 0.324, p = 0.001; OS: HR = 0.221, p < 0.001), stage IVa (OS: HR = 0.441, p = 0.047), and PSR status (OS: HR = 0.325, p = 0.007) were independent prognostic factors. Conclusion: First-line PC achieved an mPFS of 8.5 months and an mOS of 24.2 months in stage IV thymic carcinoma. The PSR group demonstrated significantly longer survival outcomes compared to the de novo group (p = 0.015). Completing 5 - 6 cycles, stage IVa, and PSR status were independent prognostic factors for OS.
