LONG-TERM SURVIVAL OUTCOMES AND PROGNOSIS OF DIFFERENTIATED THYROID CANCER WITH PULMONARY METASTASES TREATED WITH I-131

Abstract
Objectives: To evaluate long-term survival outcomes and overall survival (OS) prognostic factors in differentiated thyroid cancer (DTC) patients with pulmonary metastases treated with I-131. Methods: A retrospective and prospective descriptive study was conducted on 165 DTC patients with pulmonary metastases treated with I-131 at 108 Military Central Hospital from August 2004 to March 2026. OS rates and independent prognostic factors were analyzed using the Kaplan-Meier method and Cox regression model. Results: The mean follow-up time was 91.8 ± 49.4 months. The 5-year and 10-year OS rates from the diagnosis of pulmonary metastasis were 96.4% and 78.4%, respectively. Multivariate analysis identified three independent prognostic factors increasing mortality risk: Age ≥ 55 (HR = 10.49; p < 0.001), non-papillary histology (follicular and poorly differentiated carcinomas) (HR = 5.16; p = 0.020), and delayed metastasis detection > 6 months (HR = 3.24; p = 0.040). Conclusion: Long-term OS rate in DTC patients with pulmonary metastases treated with I-131 is relatively favorable. Age ≥ 55, non-papillary histology, and delayed metastasis detection are independent predictors of a poor prognosis.
