IMAGING CHARACTERISTICS AND DIAGNOSTIC VALUE OF MAGNETIC RESONANCE IMAGING FOR HEPATOCELLULAR CARCINOMA

Abstract
Objectives: To describe magnetic resonance imaging (MRI) features and evaluate the diagnostic value of MRI for hepatocellular carcinoma (HCC). Methods: A prospective, cross-sectional descriptive study with histopathological correlation was conducted on 196 patients with focal liver lesions who underwent liver MRI at K Hospital, Tan Trieu Campus, from June 2025 to December 2025. Results: Histopathology confirmed 122 HCCs and 74 non-HCC lesions. HCCs were commonly solitary (68.03%), located in the right lobe (61.48%) and had a median size of 4.5cm. Key findings included T2 hyperintensity (97.54%), T1 hypointensity (93.44%), DWI hyperintensity (95.90%), true diffusion restriction (78.69%), non-rim arterial phase hyperenhancement (APHE) (95.08%), delayed washout (81.15%) and a capsule appearance (51.64%). MRI had a sensitivity of 87.70%, a specificity of 87.84%, a positive predictive value of 92.24%, a negative predictive value of 81.25%, an accuracy of 87.76% and an AUC of 0.878. Conclusion: MRI demonstrated high diagnostic value for HCC. Key features include non-rim APHE, delayed washout, T2 hyperintensity, T1 hypointensity, and DWI hyperintensity/true diffusion restriction.
