BLASTOCYST OUTCOMES FROM POOR-QUALITY EJACULATED AND SURGICALLY EXTRACTED SPERM

Abstract

Objectives: To evaluate embryological outcomes of IVF cycles using poor-quality ejaculated sperm and surgically retrieved sperm from the epididymis and testis. Methods: A retrospective, descriptive study was conducted on 120 in vitro fertilization/intracytoplasmic sperm injection (IVF/ICSI) cycles performed at the Tam Anh General Hospital during the two-year period of 2023 - 2024. The cycles were divided into four groups according to sperm source and quality. Group 1 included ejaculated sperm with oligo-astheno-teratozoospermia (OAT) (n = 37); group 2 included severe oligo-astheno-teratozoospermia (severe OAT) (n = 33); group 3 included epididymal sperm retrieval (n = 34); and group 4 included testicular sperm retrieval (n = 20). Results: The fertilization rates were 85.9%, 74.3%, 79.5%, and 81.8% through 4 groups, respectively. The embryo day-3 formation rates were 87.38%, 74.68%, 79.53%, and 81.06%. The blastulation rates were 61.68%, 44.75%, 53.36%, and 55.29%, while the good-quality blastocyst rates were 26.12%, 23.72%, 28.14%, and 28.23%, respectively. Conclusion: IVF/ICSI cycles using epididymal and testicular sperm demonstrated more favorable embryological outcomes compared with cycles using severe OAT ejaculated sperm.
