A CLINICAL CASE REPORT: SEPTAL BONE UTILISATION IN THE RECONSTRUCTION OF A SEVERELY DEVIATED NASAL PYRAMID

Abstract

A deviated nasal pyramid represents a significant challenge in rhinoplasty, requiring the surgeon to comprehensively address both aesthetic deformities and functional airway impairment. We report the case of a 26-year-old male patient who presented with axial deviation of the nasal pyramid and severe left-sided nasal obstruction following a sports-related injury 14 years prior. Clinical examination and imaging studies revealed severe left-sided nasal pyramid deviation, left septal deviation, and bilateral inferior turbinate hypertrophy. The patient underwent comprehensive structural rhinoplasty with four simultaneous procedures: nasal osteotomies; septal reconstruction; dorsal augmentation with a Silastic implant; and inferior turbinate reduction. Notably, septal bone grafting was employed for septal reconstruction and stabilisation of the nasal pyramid axis. This multi-procedural approach resulted in restoration of aesthetic symmetry, significant improvement in nasal airflow, and long-term structural stability of the nasal pyramid.
