INITIAL OUTCOMES OF INTERNAL CAROTID ENDARTERECTOMY USING A TEMPORARY SHUNT UNDER SUPERFICIAL CERVICAL PLEXUS BLOCK

Abstract

Objectives: To evaluate the outcomes of internal carotid endarterectomy using a temporary shunt under superficial cervical plexus block. Methods: A descriptive case series, retrospective, non-controlled study was conducted on 14 patients with carotid artery stenosis who underwent endarterectomy at Military Hospital 103 from 2023 to 2025. Results: The mean age was 68.1 years; males accounted for 85.7%. Symptomatic carotid artery stenosis was present in 64.3%. All patients had concomitant hypertension. The unilateral cerebral ischemia time due to carotid artery clamping and the cerebral perfusion time via carotid artery shunt were 1.3 ± 1.2 minutes and 24.8 ± 4.5 minutes, respectively. 3 cases experienced loss of consciousness due to carotid artery clamping for temporary shunt placement; 1 case required conversion to endotracheal intubation anesthesia due to stimulation. There were no cases of bleeding, stroke, or death during the follow-up period. No restenosis of the carotid artery occurred within the 12-month follow-up period. Conclusion: Internal carotid artery endarterectomy is a safe surgical procedure that yields favorable outcomes for patients with a low rate of adverse events and complications.
