INVESTIGATION INTO THE VARIATIONS OF LACTATE-TO-ALBUMIN RATIO AND ITS CORRELATION WITH SEVERITY IN SEPTIC SHOCK PATIENTS

Abstract

Objectives: To investigate the variations of the lactate/albumin ratio (LAR) and its correlation with the severity as well as mortality prediction value in septic shock patients. Methods: A retrospective, observational, longitudinal descriptive study was conducted on 50 patients diagnosed with septic shock and treated at the Department of Internal Intensive Care, Military Hospital 103, Vietnam Military Medical University from October 2024 to November 2025. LAR was assessed at predefined time points: T0 (on admission), and T1 - T4 (at 24, 48, and 72 hours, and on day 5 following confirmation of septic shock). Statistical analyses were performed using the Mann-Whitney U test, Spearman’s rank correlation, and receiver operating characteristic (ROC) curve analysis. Results: The mean age was 69.7 ± 15.4 years; males accounted for 64%. At T0, the deceased group had a higher SOFA score than survivors (13.6 ± 4.8 vs. 10.4 ± 3.2; p < 0.05). Peak LAR at T0 was 2,121 (1,296 - 3,208), decreasing through the study period. The deceased group had higher LAR than survivors from T0 - T3 (p < 0.05). LAR was positively correlated with the SOFA score (r = 0.369; p < 0.01). AUCs of LAR from T0 - T3 were 0.665; 0.816; 0.715; 0.848, respectively, indicating fair to good predictive value for 7-day mortality. Conclusion: LAR was statistically higher in the deceased group from T0 - T3, positively correlated with SOFA, and demonstrated predictive value for mortality in septic shock patients.
