Abstract

Objectives: To determine the prevalence of obstructive sleep apnea (OSA) and characterize sleep-related breathing disorders using polysomnography in patients  with stable group E chronic obstructive pulmonary disease (COPD). Methods: A cross-sectional descriptive study was conducted on 60 patients with stable COPD at the Department of Respiratory Medicine, Military Hospital 175 from March 2025 to January 2026. All participants underwent clinical assessment, spirometry, and overnight polysomnography. Key variables included apnea-hypopnea index (AHI), oxygen desaturation index (ODI), mean and nadir SpO₂, and sleep architecture parameters. Results: The prevalence of COPD-OSA overlap syndrome was 53.3%, with a predominance of moderate-to-severe level. Patients with overlap syndrome exhibited significantly higher AHI and ODI, and lower SpO₂ compared to those with COPD alone. Reduced sleep efficiency was observed in 90.0% of patients with OSA. Conclusion: OSA is highly prevalent in patients with stable group E COPD and is associated with more severe sleep-related breathing disorders, worse sleep quality, and nocturnal hypoxemia.
