ASSESSMENT OF THE ASSOCIATION OF COMMON CAROTID ARTERY DOPPLER ULTRASOUND CHARACTERISTICS WITH SELECTED FACTORS AND PREDICTIVE VALUES FOR CAROTID ATHEROSCLEROSIS IN MAINTENANCE HEMODIALYSIS PATIENTS

Abstract

Objectives: To assess the association of morphological and hemodynamic parameters of the carotid artery with selected factors and predictive values for carotid atherosclerosis in maintenance hemodialysis (MHD) patients. Methods: A retrospective, prospective, cross-sectional descriptive study was conducted on 99 MHD patients at Military Hospital 103 from January to December 2025. Results: The mean common carotid artery diameter was 7.34 ± 0.95mm. The carotid intima-media thickness (IMT) was 0.87 ± 0.18mm. The prevalence of increased IMT and atherosclerosis was 56.6% and 19.2%, respectively. Peak systolic velocity (PSV) was 93.56 ± 22.51 cm/s, end-diastolic velocity (EDV) was 26.35 ± 7.7 cm/s, and resistive index (RI) was 0.74 ± 0.07. Age was positively correlated with IMT, carotid diameter, and RI, and negatively correlated with PSV and EDV (p < 0.01). BMI was positively correlated with carotid diameter and PSV (p < 0.05). Multivariate regression analysis showed that older patients, diabetes mellitus, and reduced serum albumin were independent factors associated with atherosclerosis (p < 0.05). Age and serum albumin had predictive value for atherosclerosis (AUC: 0.875 and 0.725, respectively). Conclusion: Carotid morphological and hemodynamic parameters are associated with several factors in patients on MHD. Age and serum albumin have predictive value for atherosclerosis, with age being a better predictor.
