STUDY ON SOME PROGNOSTIC FACTORS FOR COMPLICATED APPENDICITIS IN PATIENTS WITH ACUTE APPENDICITIS

Abstract

Objectives: To identify clinical and paraclinical factors with prognostic value for complicated appendicitis in patients with acute appendicitis. Methods: A cross-sectional descriptive study was conducted on 246 patients with acute appendicitis who underwent appendectomy at Military Hospital 103 from January to August 2025. Histopathological examination of appendiceal specimens was used to classify cases into complicated and uncomplicated appendicitis. Clinical and paraclinical variables were compared between the two groups, and logistic regression analysis was performed to identify factors predictive of complicated appendicitis. Results: Complicated appendicitis accounted for 26.4% of cases. White blood cell count, neutrophil percentage (%N), and CRP levels were significantly higher in the complicated group. %N and CRP were independent predictors of complicated appendicitis. A logistic regression model combining these two factors showed moderate discriminatory ability, with an AUC of 0.670 (95%CI: 0.595 - 0.745; p < 0.001). Conclusion: %N and CRP levels are independent prognostic factors for complicated appendicitis.
