STUDY ON THE RELATIONSHIP BETWEEN RIGHT VENTRICULAR-PULMONARY ARTERIAL COUPLING INDEX AND SEVERAL ECHOCARDIOGRAPHIC PARAMETERS IN PATIENTS WITH MITRAL STENOSIS

Abstract

Objectives: To investigate the relationship between the right ventricular-pulmonary arterial coupling (RVPAC) index and several echocardiographic parameters in patients with mitral stenosis. Methods: A retrospective, prospective, cross-sectional descriptive, controlled study was conducted on 100 individuals, divided into 2 groups: The research group included 58 patients diagnosed with mitral stenosis and indicated for surgery at Military Hospital 103 from October 2022 to December 2025; the control group consisted of 42 subjects undergoing routine health checkups, with no cardiovascular disease. Results: RVPAC in the mitral stenosis group (0.44 ± 0.20 mm/mmHg) was significantly lower than that in the control group (0.56 ± 0.19 mm/mmHg); p < 0.01. The RVPAC < 0.55 mm/mmHg group had significantly higher LAD (left atrial diameter) and PASP (pulmonary artery systolic pressure) compared to the RVPAC ≥ 0.55 mm/mmHg group. The group with LAD ≥ 55mm had significantly lower RVPAC than the group with LAD < 55mm (0.36 ± 0.13 and 0.48 ± 0.22 mm/mmHg, respectively; p < 0.05). No significant difference in RVPAC was observed between the groups stratified by Dd (diastolic diameter): Dd ≥ 50mm vs. Dd < 50mm or EF (ejection fraction): EF ≥ 50% vs. EF < 50% (p > 0.05).  Conclusion: RVPAC was significantly lower in the mitral stenosis group than in the control group. RVPAC is associated with several basic echocardiographic parameters, such as LAD and PASP. No association of RVPAC with Dd and EF was found in patients with mitral stenosis.
