EVALUATION OF TREATMENT OUTCOMES OF CHRONIC FUNGAL RHINOSINUSITIS USING ENDOSCOPIC SINUS SURGERY COMBINED WITH POSTOPERATIVE NASAL IRRIGATION

Abstract

Objectives: To evaluate treatment outcomes of chronic non-invasive fungal rhinosinusitis using endoscopic sinus surgery combined with postoperative nasal irrigation. Methods: A cross-sectional descriptive study was conducted on 58 patients with chronic non-invasive fungal rhinosinusitis, who underwent endoscopic surgery and postoperative irrigation at Military Hospital 103 from October 2022 to April 2024. Results: The fungal test showed positive fungal culture results of 22.41%; the fungal culture was positive in 27.59% of cases; 100% of the positive fungal culture samples were Aspergillus. Histopathological examination showed fungal hyphae in 95.1% of patients and fungal spores in 4.9%. 81.03% of patients had postoperative time ≤ 3 days. After surgery for 3 months, all signs and symptoms decreased and improved to normal, 100% with no residual fungi, 100% with wide sinus opening, and no nasal cavity adhesion. The Lund-Kennedy score was normal since 3 months post-surgery, no patients experienced recurrence of fungal sinusitis. Conclusion: Endoscopic sinus surgery combined with postoperative nasal irrigation is an effective, minimally invasive treatment method that completely eradicates fungal infection in cases of chronic non-invasive fungal rhinosinusitis.
