Abstract

Hepatolithiasis is a common disease in the Asia-Pacific region in general and in Vietnam in particular, and management strategies are quite diverse with the aim of removing the stones completely. Our clinical case involves a 41-year-old female patient with no history of chronic illness who was admitted due to right upper quadrant pain accompanied by fever a few weeks prior. The patient was diagnosed with "Biliary tract infection due to right hepatolithiasis, stricture of the right hepatic duct, and common bile duct stones". The patient underwent open hepatic parenchyma lithotomy and common bile duct exploration to remove the stones,


dilation of the right hepatic duct, and Kehr drainage. Postoperatively, the patient had a stable recovery and was discharged after 7 days of treatment. Through this case, we conclude that hepatic parenchyma lithotomy can be performed in selected cases, providing good stone clearance outcomes.
