ANALYSIS OF VANCOMYCIN UTILIZATION IN INFECTION TREATMENT AT HA DONG GENERAL HOSPITAL

Abstract
Background & Objective: Vancomycin is an antibiotic with a narrow therapeutic range, and its use needs to be optimized. Therefore, this study was conducted to analyze the usage of vancomycin in order to enhance its effectiveness at Ha Dong General Hospital. Subjects and methods: A retrospective descriptive study was conducted, based on medical records of patients who were prescribed vancomycin at Ha Dong General Hospital for the period of 01/06/2022 to 31/05/2023. Results: The study sample comprised 129 patients with a median age of 62 years. Males constituted 70.5% of the sample. A total of 78.3% of patients were discharged with recovery or improvement. Vancomycin was prescribed empirically in 60.6% of cases. All patients underwent microbiological culture, with 23 specimens yielding positive results. Staphylococcus aureus was the most common isolate, accounting for 44.2% of positive cultures. Loading dose regimen was used in 22.5% of cases. The common maintenance doses were 1g/12h (52.3%) and 0.5g/12h (32.0%). The incidence of nephrotoxicity was 3.9%. Conclusion: The proportion of patients receiving a loading dose is low, and most patients do not have their doses adjusted when renal function changes during treatment. Therefore, clinical pharmacy activities need to be strengthened to optimize the vancomycin dosing regimen at the hospital.
