Abstract

Objectives: To describe the clinical and histopathological characteristics of benign laryngeal lesions (BLL) and evaluate the outcomes of endoscopic microsurgery using the Voice Handicap Index (VHI). Methods: A prospective, longitudinal study was conducted on 84 patients who were diagnosed with BLL and underwent endoscopic microsurgery at Military Hospital 103 from June 2025 to February 2026. Clinical features, histopathological findings, and VHI scores were assessed before surgery and at 1 month postoperatively. Results: The mean age was 44.13 ± 12.48 years; 61.9% were female, and 67.8% had occupations requiring frequent voice use. Hoarseness was present in 100% of patients, most commonly with a gradual onset (81.0%), intermittent course (88.1%), and moderate severity (90.5%). Laryngoscopy showed that lesions were mainly located in the middle third of the vocal fold (51.2%) and were unilateral (64.3%). Histopathology revealed vocal fold polyps in 33.3%, vocal nodules in 31.0%, and vocal fold cysts in 11.9% of cases. At 1 month after surgery, the mean VHI score decreased from 41.67 to 4.17, and 77.4% of patients no longer had voice handicap. Conclusion: Endoscopic microsurgery for BLL markedly improved VHI outcomes.
