Mục tiêu: Mô tả phổ đột biến gene TP53, PIK3CA và phân tích mối liên quan của chúng với các đặc điểm sinh học của khối u ở bệnh nhân (BN) ung thư vú (UTV) người Việt Nam. Phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang, hồi cứu trên 139 BN được chẩn đoán xác định UTV dựa trên lâm sàng, cận lâm sàng và kết quả giải phẫu bệnh tại Bệnh viện K từ tháng 7/2025 - 4/2026. Các biến thể di truyền được xác định bằng phương pháp giải trình tự thế hệ mới (NGS) và được phiên giải theo tiêu chuẩn của ClinGen/CGC/VICC. Kết quả: 60% BN mang ít nhất một đột biến, với 14% mang đồng thời cả hai gene. Đột biến sai nghĩa chiếm ưu thế ở cả gene TP53 (60,7%) và gene PIK3CA (91,7%). Đột biến TP53 mất chức năng (loss-of-function - LOF) liên quan đến tuổi chẩn đoán trẻ hơn (trung vị 42 tuổi) và nguy cơ có chỉ số Ki67 > 50% cao gấp 2,73 lần, trong khi đó, đột biến sai nghĩa TP53 liên quan đến odds ER, PR dương tính thấp hơn so với nhóm không mang biến thể TP53. Ngược lại, đột biến PIK3CA miền Helical liên quan chặt chẽ đến việc tăng bộc lộ thụ thể nội tiết estrogen (ER) và progesterone (PR). Kết luận: Các biến thể gene TP53 và PIK3CA có mối liên quan mật thiết đến kiểu hình sinh học của khối u.
