KẾT QUẢ PHẪU THUẬT ĐIỀU TRỊ U ĐẶC GIẢ NHÚ THÂN, ĐUÔI TỤY TẠI BỆNH VIỆN HỮU NGHỊ VIỆT ĐỨC

Mục tiêu: Đánh giá và theo dõi xa kết quả sớm phẫu thuật điều trị u đặc giả nhú (solid pseudopapillary neoplasms - SPNs) thân, đuôi tụy tại Bệnh viện Hữu nghị Việt Đức. Phương pháp nghiên cứu: Nghiên cứu hồi cứu, mô tả, theo dõi dọc trên 39 bệnh nhân (BN) được chẩn đoán SPNs ở thân, đuôi tuỵ, được phẫu thuật tại Bệnh viện Hữu nghị Việt Ðức từ ngày 01/12/2014 - 30/11/2019. Kết quả: Tuổi trung bình là 27,3; nữ chiếm 94,9%; kích thước u trung bình là 57,8mm. Phẫu thuật nội soi (28,2%), mổ mở (71,8%), đa số BN được cắt thân, đuôi tụy và cắt lách là 61,5%. Theo dõi xa được 35 BN, không có BN nào tử vong, 2 BN tái phát sau 37 và 50 tháng sau mổ. Hầu hết có chất lượng cuộc sống tốt (85,7%), tỷ lệ sống thêm 5 năm không bệnh đạt 94,2%. Kết luận: Phẫu thuật điều trị SPNs thân, đuôi tụy cho kết quả tốt, tỷ lệ tái phát thấp, chất lượng cuộc sống tốt. Phẫu thuật nội soi (PTNS) cắt thân, đuôi tụy điều trị SPNs được thực hiện an toàn và khả thi. Các yếu tố giới tính, nồng độ CA 19-9, kích thước và cấu trúc u chưa thấy có mối liên quan đến tái phát.
